Predictors of complications in venomous snakebites

Objectives: Factors responsible for causing complications in venomous snakebites are unknown. The present study was planned to identify such factors. Material and Methods: This a prospective observational study. All patients presenting the emergency department between April 2016 August 2017 with meeting inclusion/exclusion criteria were enrolled. Patients divided into two groups based on presence/absence of complications. Risk factors associated analyzed. Results: Out 236 screened, 156 eligible. Mean age 39.5 ± 15.6; majority ( n = 122, 78.2%) group 20–59 years; 119 (76.3%) males. Eighty-eight (56.4%) developed complications, which, cellulitis/compartment syndrome 54, 34.6%) most common, followed by acute kidney injury 18, 11.5%), coagulopathy/disseminated intravascular coagulation 17, 10.9%), neuroparalysis needing ventilator 12, 7.7%), shock 8, 5.1%), respiratory distress (three patients, 1.9%), sepsis (one patient, 0.6%). mortality rate 1.92%. On univariate analysis, low hemoglobin, elevated neutrophil count, lower lymphocyte serum creatinine, initial/total dose anti-snake venom, prolonged hospital stay had statistically significant association multivariate count (OR 1.084; 95% CI 0.997, 1.179) 1.975; 1.393, 2.800) Bite-to-needle time mortality. Conclusion: nature depends composition thus varies geographical region. Association counts points an inflammatory hypothesis. bite-to-needle highlights early use antivenom.
